Video Gaming Terminal (VGT) Applications
The process for all VGT applications will be as follows:
Once an application is submitted, the applicant will receive instructions for how to be fingerprinted.
After the PGCB receives the fingerprint results and all other deficiencies in the application are cured, the application will be deemed complete.
The completed application for each individual and entity will undergo a background investigation.
Enterprise Entity Application and Disclosure Information Form 
Entities applying for a Platform Operator, Manufacturer, or Supplier license must complete this form. An intermediary or holding company of an applicant or licensee must also complete this form.
Abbreviated Application for Licensed Manufacturers to apply for a Video Gaming Manufacturer License
Entities currently licensed as a Manufacturer with the Pennsylvania Gaming Control Board may complete this form.
Abbreviated Application for Licensed Suppliers to apply for a Video Gaming Suppliers License
Entities currently licensed as a Supplier with the Pennsylvania Gaming Control Board may complete this form.
Multi-Jurisdictional Personal History Disclosure Form
Individuals applying as a Principal/Key employee must complete the form.
Principal/Key Employee Form-Pennsylvania Supplement to the Multi-Jurisdictional Personal History Disclosure Form 
Individuals applying as a Principal/Key employee must complete the form.
Abbreviated VGT License RENEWAL Application (Not for Main Entity)
This abbreviated VGT license renewal application may be used by an entity or individual currently licensed with the Board as an Affiliate, Principal Entity, Principal, or Key Employee and they hold other non-VGT licenses that are not required to renew at this time.
Principal Entity Form
An entity that is otherwise required to be licensed as a Principal and is not an intermediary or holding company of an applicant or licensee must complete this form.
VGT Establishment License Application
Truck stop establishments must complete this form.
Procurement Agent Entity Application and Disclosure Information Form
